Dr. Hancock founded the clinic in Black Canyon City in 1999 to give the community a place to get care regardless of ability to pay. He continues to see patients and leads the health center's medical staff.

Matt earned his bachelor's degree from Tabor College in 2015 and his Master of Science in Physician Assistant Studies from A.T. Still University. He returned to Black Canyon Community Health Center in 2021 and focuses on whole-person, evidence-based family medicine.

Jodi was born and raised in Arizona and has practiced primary care for nearly ten years. She treats every patient as an individual and works with them as a partner toward their best health. She is married with three daughters.

Isabella is an Arizona native. She earned a bachelor's degree in biology from the Grand Canyon University Honors College and a Master of Science in Physician Assistant Studies from A.T. Still University. She provides whole-person care to Black Canyon City and nearby communities.

She earned her Master of Physician Assistant Studies from Northern Arizona University and values lasting relationships with patients and families.
